Amazon's 2026 Logistics Strategy: Expanding Same-Day Pharmacy Delivery and Diversifying Transport Modes

Amazon's digital pharmacy business has just announced plans to expand its same-day prescription delivery service to an additional 2,000 cities and towns across the United States by the end of 2026. This is part of the corporation's broader initiative to accelerate delivery speeds across all product categories, getting medications into the hands of millions of customers as quickly as possible.

Record Delivery Speeds in 2025

Last week, Amazon announced a record for delivery speeds in 2025, with over 13 billion items delivered the same or next day globally. In the U.S. alone, Prime members received more than 8 billion items within this timeframe, a 30% increase year-over-year. Notably, groceries and everyday essentials accounted for half of this total volume.

Amazon stated that its fast, free delivery service saved U.S. Prime members tens of trips to physical stores over the past year.

Covering 4,500 Areas and Solving "Pharmacy Deserts"

Once fully rolled out by the end of this year, Amazon Pharmacy will offer free same-day delivery to nearly 4,500 areas. The newly served regions will include states like Idaho and Massachusetts, where Amazon notes that pharmacy closures, staffing shortages, and transportation barriers have historically limited access to healthcare.

John Love, Vice President of Amazon Pharmacy, emphasized in a press release: “Patients shouldn’t have to choose between speed, cost, and convenience when it comes to their medication, regardless of where they live. By combining our pharmacy expertise with our logistics network, we’re removing critical barriers and helping patients start treatment faster - setting a new standard for accessible, digital-forward pharmacy care.”

Multimodal Logistics: From E-bikes to... Horses

Amazon Pharmacy customers experienced significant delivery improvements in 2025 across various communities, thanks to flexibility in last-mile transport modes:

•    In high-density urban areas like Manhattan, goods are delivered via e-bikes.

•    In suburban areas like Chesterbrook, Pennsylvania, Amazon utilizes electric vehicles.

•    In remote locations like Mackinac Island, Michigan, prescriptions are transported by ferry and horse.

Additionally, in Los Angeles, One Medical patients can now pick up medications within minutes using Amazon Pharmacy Kiosks located directly in the lobby before leaving the clinic, allowing treatment to begin immediately. Amazon confirmed it will continue to expand direct kiosk access to more locations in 2026.

Typically, Amazon Pharmacy customers receive medications the next day or within two to three days.

Investing in Rural Infrastructure and the Micro-Fulfillment Model

Last year, Amazon expanded the geographic reach of its same-day and next-day delivery service to over 4,000 small cities and rural areas across 44 states. This was the result of a $4 billion investment to build out its rural delivery network.

In December, Amazon stated it had met its goal of offering same-day delivery for fresh groceries to 2,300 cities and towns by year-end, more than doubling its previous reach. The company also began testing 30-minute delivery in Seattle and Philadelphia through the use of micro-fulfillment centers.

The Prime membership program first launched in 2005, offering free two-day delivery on a selection of one million items, primarily DVDs, CDs, and books. Today, members have access to free delivery on over 300 million items across 35 different categories.
